Aims of Module
To develop successful approaches to practice that ensure the safe, effective and efficient use of medicines through collaborative working.

Learning Outcomes for Module
On completion of this module, students are expected to be able to:
1 Critically evaluate and reflect on the structures and processes required to ensure the provision of effective healthcare.
2 Systematically explore and evaluate the critical aspects that support continuity of care.
3 Critically reflect on team-working skills required for effective collaborative working.

Indicative Module Content
Team-working skills, including leadership and management skills, to support effective collaborative working. Critical aspects that support continuity of care, including patient safety and medicines reconciliation, between health and social care settings. The structures and processes, including clinical governance and evidence based practice, required to achieve safe and effective medicines management.

Module Delivery
Online delivery; self-directed learning supported by web-based learning materials, peer to peer learning, participation in synchronous and asynchronous webinars and discussion fora.
