Aims of Module
To provide students with the ability to systematically synthesise, implement, and evaluate care plans for individual patients.

Learning Outcomes for Module
On completion of this module, students are expected to be able to:
1 Collate and critically review patient information from a wide variety of sources within the context of pharmaceutical care planning.
2 Synthesise and justify pharmaceutical care plans using principles of evidence based medicine and a holistic approach.
3 Critically evaluate models of consultation in relation to practice.
4 Reflect on own learning in practice in the context of pharmaceutical care provision.

Indicative Module Content
Accurate and effective communication, building and maintaining effective relationships, health literacy, consultation models. Medication review in patients with multiple pathology and multiple drugs, identification, categorisation and documentation of care issues and actions. Relevant sources, utilisation and limitations of patient information. Cultural and religious beliefs. Equality, diversity and inclusion. Evidence based practice. Polypharmacy, deprescribing. Care plans, documentation, systematic implementation and evaluation, monitoring response, modifying treatment. Legal, regulatory, ethical and professional frameworks. Documentation, professional responsibility. Professional relationships and communication with healthcare professionals and patients. Reflective practice.
