Aims of Module

To provide the student with the ability to systematically synthesise, implement and evaluate pharmaceutical care plans for individual patients in primary and secondary care.

Learning Outcomes for Module

On completion of this module, students are expected to be able to:

1. apply knowledge and understanding of physiology, pathology, pharmacology and therapeutics to pharmaceutical care plan synthesis.
2. retrieve information from a wide variety of sources (including directed reading, CAL packages, video presentations and standard medicines information courses) and incorporate this information into pharmaceutical care plans.
3. retrieve and utilise patient information from a wide variety of sources (including case notes, patient medication records and patient interviews) to synthesise and defend pharmaceutical care plans.
4. communicate effectively using relevant professional and interpersonal skills and work effectively as a member of a a team.

Indicative Module Content

Therapeutic update on disease states of major body systems, eg. nervous system, cardiovascular system.
General aspects of clinical pharmacy and pharmaceutical care: selection of appropriate drug therapies for individual patients; monitoring of drug therapy for efficacy and toxicity; interpretation of clinical data; structure and use of case notes, medical problem lists and pharmaceutical care plans. Application of the above to special groups including terminally ill, elderly, mentally ill, paediatrics, pregnant and breast feeding mothers.

Lectures, coursework sessions (to include individual and group problem solving exercises, tutorial sessions, clinical visits and use of CAL materials) and managed student centred learning activities.
