Prerequisites for Module
An honours undergraduate degree in a pharmacy, medical or nursing & midwifery related field from a UK university or overseas equivalent, and one year post-registration experience OR an integrated masters degree OR in consultation with the Course Leader.

Aims of Module
To apply skills of pharmaceutical care planning to individual patients within specialised clinical areas in secondary care.
Learning Outcomes for Module
On completion of this module, students are expected to be able to:

Contextualise pharmaceutical care provision in secondary care through consideration of effective management of resources.

Show evidence of professional ability in the systematic application of knowledge and understanding of physiology, pathology, pharmacology and therapeutics to care plan synthesis in patients with complex pharmaceutical needs within specialised clinical areas in secondary care.

3. |
Systematically retrieve relevant information,for patients in specialist areas, from a wide variety of sources and incorporate this information into evidence based care plans that address complex pharmaceutical needs in secondary care.

Synthesise, document, justify and reflect on evidence based care plans including monitoring and evaluating patients' response to therapy and modifying treatment accordingly in patients within specialised clinical areas in secondary care.

| Indicative Module Content
Pharmaceutical care planning and medication review for patients in specialised clinical areas for example, psychiatry, neurology, heamatology and oncology, paediatrics, frail elderly, palliative care, critical care, hepatic and renal disorders and surgery. In the context of care planning: performance management / development, performance planning and decision making.
