Aims of Module
To apply skills of pharmaceutical care planning to individual patients within specialised clinical areas.
Learning Outcomes for Module
On completion of this module, students are expected to be able to:

Apply knowledge and understanding of physiology, pathology, pharmacology and therapeutics to care plan synthesis in patients within specialised clinical areas.

Retrieve information relating to case studies from a wide variety of sources and incorporate this information into evidence based care plans in patients within specialised clinical areas.

Synthesise, document, justify and reflect on evidence based care plans including monitoring and evaluating patients' response to therapy and modifying treatment accordingly in patients within specialised clinical areas.

Indicative Module Content
Pharmaceutical care planning and medication review for patients in specialised clinical areas for example, paediatrics, frail elderly, mental illness, palliative care, critical care, hepatic / renal disorders.
