Aims of Module
To enable the student to develop a critical understanding of the theories, principles and concepts within contemporary community nursing.

Learning Outcomes for Module
On completion of this module, students are expected to be able to:
1 Critically explore theories, principles, and concepts which underpin improving health and wellbeing.
2 Evaluate theoretical concepts of person-centred assessment and professional decision making within advancing community nursing practice.
3 Critically analyse the theoretical basis of a range of strategies and approaches to addressing healthcare needs within advancing community nursing practice.
4 Critically explore the complexity and influences associated with providing care in the community setting.
5 Appraise the underpinning principles and concepts of managing teams and caseloads.

Indicative Module Content
Assessment frameworks; Carers, person-centred; Interprofessional relationships and collaborative working; Integrated health and social care; Case management; Caseload management; Long term conditions, palliative care, end of life care, self-care, empowerment; Health promotion and public health; Inequalities, equity, culture and diversity, social prescribing, behavioural changes, strengths based approaches, ethical and legal considerations; Risk assessment and management; Human factors, vulnerability; Acute care in the community; Complex care.
