Aims of Module
To provide a holistic theoretical knowledge base for the student to enable them to develop their role in participating in the management of individuals with long term conditions.

Learning Outcomes for Module
On completion of this module, students are expected to be able to:
1 Critically examine the impact of local and national political, health and social care agendas on the management of long term conditions.
2 Analyse and evaluate approaches for holistic assessment of patients relating to managing long term conditions.
3 Explore and critically review current therapeutic interventions set within a multi professional and interagency context.
4 Justify a comprehensive and in depth knowledge of emerging management techniques and strategies for long term conditions.
5 Critically evaluate psychosocial concepts and strategies aimed at supporting patients, families and carers in the management of their long term condition/s.

Indicative Module Content
National and local policy in health and social care relating to long term conditions; Epidemiology; Case finding; Risk prediction; Models and frameworks for long term condition management; Assessment frameworks; Pathophysiology and clinical management for selected long term conditions e.g. diabetes, coronary heart disease, cancer, chronic obstructive pulmonary disease, depression, dementia; Psychosocial perspectives and strategies e.g. motivational interviewing; Communication and therapeutic interventions; Medicines management; Health improvement; Self care and care management; Lead complex case co-ordination; Multidisciplinary and inter-agency working; eHealth; Telehealthcare; Managing care at the end of life; Reflective practice.

Module Delivery
The module will be delivered online via CampusMoodle. This includes discussion forums, quizzes, videos, Panopto and Blackboard Collaborate activities.
