Aims of Module | |||

To provide a holistic theoretical knowledge base for the student to enable them to develop their role in participating in the management of individuals with long term conditions. |
Learning Outcomes for Module | |

On completion of this module, students are expected to be able to: | |
1 | Critically examine the impact of local and national political, health and social care agendas on the management of long term conditions. |
2 | Analyse and evaluate approaches for holistic assessment of patients relating to managing long term conditions. |
3 | Explore and critically review current therapeutic interventions set within a multi professional and interagency context. |
4 | Justify a comprehensive and in depth knowledge of emerging management techniques and strategies for long term conditions. |
5 | Critically evaluate psychosocial concepts and strategies aimed at supporting patients, families and carers in the management of their long term condition/s. |
Indicative Module Content |

National and local policy in health and social care relating to long term conditions; Epidemiology; Case finding; Risk prediction; Models and frameworks for long term condition management; Assessment frameworks; Pathophysiology and clinical management for selected long term conditions e.g. diabetes, coronary heart disease, cancer, chronic obstructive pulmonary disease, depression, dementia; Psychosocial perspectives and strategies e.g. motivational interviewing; Communication and therapeutic interventions; Medicines management; Health improvement; Self care and care management; Lead complex case co-ordination; Multidisciplinary and inter-agency working; eHealth; Telehealthcare; Managing care at the end of life; Reflective practice. |
