| 1 | Assess the health and social needs and risk factors for clients and social groups reviewing the tools and processes involved. |

| 3 | Collaborate and co-operate within an inter-disciplinary and multi-agency team using effective communication skills. |

| This module is essential for the SCPHN - Health Visiting/ School Nurse route of the BSc(Hons) Public Health Nursing course. A minimum of 10 weeks whole time equivalent will be spent in practice. |
