Aims of Module | |||

To provide a holistic theoretical knowledge base to enable the student to develop their role in participating in the management of individuals with long term conditions. |
Learning Outcomes for Module | |

On completion of this module, students are expected to be able to: | |
1 | Examine the national and local political, health and social care agendas relating to a selected long term condition. |
2 | Analyse approaches for holistic assessment of individuals relating to managing a long term condition. |
3 | Critique a long term condition whilst reviewing current therapeutic interventions set within a multi professional and interagency context. |
4 | Analyse psychosocial concepts and strategies aimed at empowering individuals, families and carers to support self care and management. |
5 | Review the literature on emerging management techniques and strategies for a long term condition. |
Indicative Module Content |
---|
National and local policy in health and social care relating to long term conditions; Models and frameworks for long term condition management; Epidemiology, pathophysiology and clinical management for selected long term conditions e.g. diabetes, coronary heart disease, chronic obstructive pulmonary disease; Psychosocial perspectives and strategies; Medicines management; Therapeutic interventions; Health improvement; Self care and self management; Leading complex care co-ordination; Multidisciplinary and inter-agency working; Managing care at the end of life; Reflective practice. |
